Pregnant women<\/strong> are another vulnerable group. It is not uncommon for pregnant women, especially in the later stages of pregnancy, to seek help from their doctor for painful piles. This is due to increasing pressures exerted by the foetus within the pelvis and impeding venous blood flow.<\/p>

Other less likely risk factors include individuals with a history of liver and heart disease, and individuals who sit or stand for prolonged periods. The symptoms for haemorrhoids are varied and include:<\/p>

  1. Itch around the anus<\/li>
  2. Passage of blood during bowel movements<\/li>

  4. Moist mucus on the underwear despite cleaning properly after visiting the toilet<\/li>
  5. Lumps felt around the anus, especially after bowel movement<\/li>
  6. Painful lumps that need to be pushed back into the anus<\/li><\/ol>

    Haemorrhoids are generally classified as internal or external.<\/p>

    External haemorrhoids<\/strong> are most often felt as bulges or lumps at the anus and tend to be persistently present without much discomfort. When thrombosis occurs or blood clots form within them (\u2018perianal hematoma<\/strong>\u2019), the pain can be very acute and severe. The haemorrhoid becomes a dark, hard lump and extremely sensitive to touch. When this happens, patients often report to the doctor with intolerable pain on sitting and walking.<\/p>

    Internal haemorrhoids<\/strong>, conversely, are hidden within the anus, especially when the size remains small. Patients tend to experience painless bleeding<\/strong> during bowel movement as the only symptom. When internal haemorrhoids enlarge, they typically descend during bowel movement, resulting in discomfort.<\/p>

    Patients instinctively attempt to push them back in when cleaning themselves. When the piles enlarge further and are unable to be pushed back in, they may become prolapsed<\/strong> and swollen. This again results in intolerable pain and swelling that requires immediate attention from the doctor.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Typically, the doctor will recommend rubber band ligation<\/strong>, which can be performed in the outpatient setting. A rubber band is deployed tightly around the base of the haemorrhoid to strangulate the blood supply. As the haemorrhoid detaches, it leaves behind a small wound that heals with scarring.<\/p>

    Beyond medication and rubber band ligation, surgery offers a more definitive treatment option. The most common procedure is conventional haemorrhoidectomy<\/strong>, where the piles are surgically removed using diathermy or an advanced bipolar device (less pain).<\/p>

    Another technique is stapled hemorrhoidectomy,<\/strong> which acts like a \u2018nip and tuck\u2019 procedure. The haemorrhoids are removed through the firing of a stapling device, which simultaneously draws excess mucosa upwards to counteract the prolapse.<\/p>

    Both surgical procedures are performed under general anaesthesia and patients will require a period of painkillers and special care instructions after surgery. Discuss with your surgeon the various treatment options, including the benefits and risks of surgery.<\/p>
